Schedule a Physical Evaluation

Please be sure to have a current physical form before the first day of practice uploaded to your RSchoolToday.com account to insure you will be able to participate. 

Having trouble getting an appointment with your doctor?   You can get a sports physical at any Urgent Care or Little Clinic (King Soopers).  No appointment is necessary.  Check each location for times of availability.  

D11 Physical Form - Please print and upload to your student's RSchoolToday.com account once signed by the Physician.
