Please be sure to have a current physical form before the first day of practice uploaded to your RSchoolToday.com account to insure you will be able to participate. 

Having trouble getting an appointment with your doctor?   You can get a sports physical at any Urgent Care or Little Clinic (King Soopers).  No appointment is necessary.  Check each location for times of availability.  

D11 Physical Form - ***New Physical Form that must be used.  If you are getting a physical for your child starting May 2024, this new physical form is the one that needs to be used.  We will NOT ACCEPT the Doctors forms or previous D11 physical forms starting in the 2024-25 school year. If you have a current physical on file, that physical is fine, but once it expires and you need to get a new one, the new physical form needs to be used. 

 Please print and upload only Page 4 of the Physical  to your student's RSchoolToday.com account once signed by the Physician. The first three pages are yours or your physicians to keep.
